I n order to appreciate the spatiotemporal relation between the mitral leaflets and mitral annular plane during the cardiac cycle, one has to begin with the ‘‘neutral’’ position of the mitral leaflets in relation to the mitral annular plane. This can be defined as the position of the mitral leaflets when there is no pressure difference between the left atrium and left ventricle. If the annulus were a Euclidean plane, the neutral position of the leaflets would be in the plane of the annulus (fig 1A). Because the mitral annulus is saddle shaped the neutral position of the mitral leaflets is not in the Euclidean plane but is caudal to it (fig 1B). This position results from the tension on the leaflets by the saddle shaped annulus which affords them a spring-like quality so that a greater force is required to move them cephalad towards the mitral annular plane than to move them caudally further into the left ventricular (LV) cavity (fig 1B).

The Echocardiographic Determination of Mitral Valve Opening and Closure
The ability of the echocardiogram to define mitral valve opening and closure was assessed by simultaneously recording the echocardiogram of the anterior leaflet (ECHO) with intracardiac pressures, aortic second sound, and ECG on 38 occasions in 14 patients undergoing cardiac catheterization. Hemodynamic opening and closure were defined by intersection of the pulmonary wedge and left ventricular...
متن کاملMotion of both mitral valve leaflets: a cineroentgenographic study in intact dogs.
Motion and position of both mitral leaflets were studied in five normal dogs 1-11 wk after radiopaque markers were sutured on the valve cusps and on the mitral annulus. Cinefluorograms and cineangiograms (100-120 frames/s) of left atrium and left ventricle were used to study cusp motion and intraventricular flow patterns, and to detect mitral regurgitation during sinus rhythm (42-184 beats/min)...

متن کاملThe echocardiogram of the anterior leaflet of the mitral valve. Correlation with hemodynamic and cineroentgenographic studies in dogs.
The echocardiogram of the anterior leaflet of the mitral valve (ECHO) was compared to hemodynamic and cineroentgenographic data to evaluate its accuracy in timing mitral valve (MV) opening and closure, and to validate it as an indicator of MV motion. The ECHO, high speed cineroentgenography at 250 frames/sec, and/or measurement of intracardiac pressures allowed accurate timing of the events of ...
متن کاملRestricted diastolic opening of the mitral leaflets in patients with left ventricular dysfunction: evidence for increased valve tethering.
OBJECTIVES We tested the hypothesis that patients with incomplete systolic mitral leaflet closure (IMLC: apically displaced coaptation) also have restricted diastolic leaflet opening that is independent of mitral inflow volume and provides evidence supporting increased leaflet tethering. BACKGROUND Competing hypotheses for functional mitral regurgitation (MR) with IMLC include global left ven...
